Q) What am I supposed to do if we have an incident report?

A) If you are injured on the job, follow these steps:

  1. GET MEDICAL CARE. For life threatening injuries, go to the nearest emergency room or trauma center for immediate care.
  2. For routine injury treatment during normal working hours, free care is available at the Campus Health Service, located at the NW corner of Cherry Ave. and University Blvd.
  3. MAKE EARLY NOTIFICATION to the state by calling toll free 1-800-837-8583. This is a recording system that can be called 24 hours a day, seven days a week.
  4. NOTIFY YOUR SUPERVISOR AS SOON AS POSSIBLE. Ask them to complete a Supervisor's Report of Employee Injury.
  5. Workers' Compensation claims need to be reported to Risk Management within seven calendar days.
